2024-2025 Online Physicals  

Parents:   

1.  Your child's physical is NOT COMPLETE until you esign all the forms on ATS, and you upload section 6 of the physical packet.    

Please DO NOT upload your child's immunizations to this site.  The document will be deleted.  Immunization records must be turned in to the school nurse. 

Below you will find sign on info, as well as a short tutorial on how to use the system.  PLEASE WATCH THE TUTURIAL.  99% OF THE QUESTIONS YOU MAY HAVE ARE ANSWERED IN THE TUTORIAL.
